Refer a patient for CAR-T at UHS (Lymphoma)
1) Identify patient as potentially eligible (e.g. clinic/MDT)
Please refer to cancer drugs fund list for current eligibility criteria.
We would recommend discussing on phone/email with UHS lymphoma consultant prior to referral
2) Complete referral letter (optional template)
3) Email referral letter and completed NCCP form to
UHS lymphoma consultant
cc.
4) Refer patient for discussion to UHS lymphoma MDT (if not already done), by emailing brief summary to:
